We may contact you directly if you have made your initial enquiry via the Well Being Centre Southampton and your details have been passed on to us for booking.
We will give you an idea of provisional availability depending upon your preference of location, whether the assessment is face-to-face or remote and the type of assessment.

We can hold the provisional appointment date for 48 hours and will ask that completed forms be submitted during this period for the booking to be confirmed. If we do not receive the booking and GP consent form within 48 hours, we cannot hold the appointment and will need to cancel the provisional slot offered. We will be happy to offer an alternative slot once the forms have been received.
The reason we require these forms before we can confirm the booking is:
Enter your details on our electronic records and booking system to generate a confirmed appointment Document consent for compliance purposes
Email you the information we are expected to provide prior to the appointment
Have an agreement on how we communicate with you Send you the video link for the appointment If funding via PMI, match your pre-authorisation codes in the system for invoicing purposes
Request and review GP records prior to the appointment if we do not have these already
Once the forms are received and your details have been entered into our clinical records system, you will receive booking confirmation via Carebit, our electronic records system. This will include a video link if your appointment is remote.
At this stage, we will send you pre-appointment clinical questionnaires and request that you complete and return them before the appointment.
Please note that all self-pay invoices are payable within a week of the receipt of the invoice. If a payment is not received at least 48 hours before the appointment, our system will automatically cancel the booking. For Private Medical Insurance appointments, we will invoice your PMI provider after the appointment. However, you remain liable for any excess on your account and full payment at self-pay rates if your PMI provider declines to pay or if the appointment is not attended or cancelled at less than 48-hour notice.
